Amy Alexander

AMY ALEXANDER MD, MHSc, CCFP

CAPM DIPLOMATE

Amy is a Family Physician with focused practices in Psychotherapy and Pain Management. She treats patients with complex illness and pain with a combination of Western medicine, psychotherapy, medical acupuncture and yoga therapy. She is an Associate Staff member in Psychosocial Oncology at Southlake Regional Cancer Centre in Newmarket.

Robert Banner

Robert Banner, MD is Board Certified in Family Medicine and Anesthesiology and holds certification in; Functional Medicine., Traditional Chinese Medicine Acupuncture, Integrative and Holistic Medicine, Clinical Homeopathy and since 2006 the American and Canadian Academies of Pain Management . He is a member of many professional organizations and holds qualification in Ozone and Prolozone therapies. He is a founding board member of the Canadian Integrative Medical Association.

Joel Bordman

JOEL BORDMAN MD, DAAPM, CISAM

Dr. Bordman graduated from the University of Toronto Medical School in 1985. He has achieved credentials in both addiction and pain from international societies. He is a mentor to other doctors as part of The College of Family Physicians of Ontario’s “Medical Mentorship in pain and addiction” program (MMAP). He has experience in Emergency Medicine, Palliative Care, Family Practice and Long Term Care. His main interest is in treating Opioid Addiction and assessing Chronic Pain treatment in the complex patient.

John P. Crawford

A graduate of CMCC ‐ Toronto, Canada, Dr. Crawford is a Fellow, Royal College of Chiropractic Sports Sciences (Canada), Vice‐President, Royal Sports Foundation of Canada, an Independent Examiner with FSCO, fully certified through the ABFP in the AMA Guides (4th Ed), Board Member ‐ CSCE and CAPM (Advanced Credentialed Member), and certified in concussion‐ related disorders and vestibular rehabilitation.

Hany Demian

HANY DEMIAN MD, MBBCH, MCFP, CEUSIP

Dr. Hany Demian has been practicing for 16 years. He is the co‐founder, medical director of Pain Care Clinics one of the largest pain clinics in Ontario. He is a CPSO peer assessor and an OHP assessor, CFP examiner, Instructor for ultrasound guided procedures.

Gloria Gilbert

A physiotherapy graduate of McGill and Western Universities, Gloria owned the first private physiotherapy in London, The Downtown Clinic (est. 1982). Although no longer accepting new clients, Gloria is currently working on web‐based educational initiatives for the person with chronic pain. Gloria is a member of the Western Interdisciplinary Pain Program and has been credentialed as a Fellow of AAPM and CAPM since 2007.

Chris Gioshev

Chris Giorshev is the medical director of an interventional pain clinic. He is an auditor at the CPSO and was a task force member of the CPSO’s OHPIP. He is the Chair of the OMA Section on Chronic Pain. He is a diplomate and on the board of the CAPM.

Eleni Hapidou

ELENI HAPIDOU PH.D., C. PSYCH

Eleni Hapidou is a psychologist at the interdisciplinary Chronic Pain Program (recently the Michael G.DeGroote Pain Clinic) and an Associate Professor in the Department of Psychiatry and Behavioral Neurosciences, McMaster University. She was credentialed by CAPM in 2014. She was previously the Treasurer and now the Vice‐President of CAPM.

Mangesh Inamdar

MANGESH INAMDAR MD, CCFP (EM), FCFP

Dr. Inamdar graduated in 1999 after completing a 4 year residency at McGill University. He currently practices interventional pain medicine at Pain Care Clinics in Oakville. He is an assistant clinical Professor at McMaster University and is also on active staff at Oakville Trafalgar Memorial Hospital. As an independent medical examiner, Dr. Inamdar holds certification through the American Board of Forensic Professionals in the AMA Guides to the Evaluation of Permanent Impairment (4th Edition). He has also appeared in Readers Digest, the Toronto star and television.

Kevin Jones

Kevin Jones is the Founder and Director of Burlington Psychological and Counselling Services. Dr. Jones works with adults and adolescents in the area of clinical, health and rehabilitation psychology. He has a special interest in CBT for chronic pain, depression, anxiety disorders, brain injury, and motor vehicle accident rehabilitation.

I graduated from the University of Baghdad, where I initially trained as a trauma surgeon, I then moved to Britain, where I received my FRCS and worked as an Emergency Physician before specializing in general practice. I started practicing in the field of pain medicine since 2012 and I have major interest in ultrasound guided nerve blocks and comprehensive approach to pain management. I currently practice as a Family physician with chronic pain management.

Shaun Segal

I am the medical director of two large multidisciplinary clinics and a level II OHP Pain Clinic in Toronto. Igor Steiman

IGOR STEIMAN MSc, DC, FCCS(C)

Dr. Igor Steiman completed his M.Sc. at University of Toronto before graduating as a chiropractor from the Canadian Memorial Chiropractic College (CMCC) in 1981. He became a Fellow of the College of Chiropractic Sciences (Can.) in 1986. His practice includes medicolegal assessments and consultations. He has served as CMCC faculty since 1981 and as Staff Chiropractor at St. Michael's Hospital, Toronto since 2004. He was credentialed with CAPM in 2015.

Michael Surkont

Dr. Michael Surkont is the Medical Director of the Vaughan Pain Clinic. He has many years of experience in providing comprehensive pain care to his patients. He possesses Advanced Credentialing in Interventional Pain Medicine by the Canadian Academy of Pain Management. He has extensive experience teaching new pain physicians and is an Adjunct Professor at Western University. He is a Peer Assessor for the College of Physicians and Surgeons of Ontario. He also serves as an Investigating Coroner for the Province of Ontario. Due to his expertise, Dr. Surkont is sought out by lawyers to provide expert opinions on complex cases involving insurance litigation.
